Mic input and Speaker input Simultaneously

Possibly another stupid question, but is it possible, when streaming your desktop, to have your mic and speakers live at the same time?

I notice when I'm watching people play games, the games music/sfx come through as the player is talking. Any way to do that with desktop?

If it's not possible, then perhaps this can be an update somewhere down the line.
