Random Video deletion?

Here's the set up:
I uploaded a video(1) with a specific name(a)
I finished it, saved it, it's on the list of other videos
I just recently finished video(2) with name(b)

Here comes the prob.
Instead of the video(2) saving, video(2) got deleted and name(b) was placed on top of video(1).

I went through the studio to find video(2) but nothing was listed.

Why?
